NEW PATIENT FORMS

To save you time on your first visit, fill out the New Patient Forms in their entirety. If you are unable to complete these before your visit, we will have them in the office for you to fill out when you arrive. Make sure to bring your photo ID and insurance card, if you have it, to that visit. Your first visit should take no more than 1 hour. If you have any questions prior to your appointment, give us a call and we will be happy to assist you.

While walk-ins are welcome, we suggest making an appointment for your first visit so that we can do our best to be respectful of your time.

If x-rays are taken, we will take the time to explain your x-rays, what is causing your problem, and how we can treat it. After reviewing everything with you and answering all of your questions, the doctor will perform your treatment.
